Ineffective lookouts contributed to boxship collision

An independent investigation into the collision last year of the Australian fishing vessel Allena and the container ship Northern Fortune, found that the lookouts on board both vessels were "ineffective."

The investigation found that the watch keepers on both vessels did not see the other vessel in "sufficient time" to avoid the collision off Bowen in Queensland on January 21.
