ATSB flags shortcomings after boiler flashback

A boiler flashback on board the Malta-flagged bulk carrier Saldanha last November had revealed manufacturer-information and crew-training shortcomings, according to an Australian Transport Safety Bureau report released today.

The shortcomings included a lack of training for the crew on boiler burners and that the fact that the oil firing unit on the Volcano VJ model in question could be modified.
