A new investigation report from the Australian Transport Safety Bureau has concluded that an incorrectly assembled nozzle caused an explosion on products tanker Qian Chi.

An oil-fired thermal oil heater exploded aboard the Hong Kong-flagged Qian Chi (IMO: 9262417), which was at anchor in Moreton Bay, Queensland just after just after 3pm on January 16, 2011.
